Recertification Document Checklist
The following information is required to complete your Housing Choice Voucher/Project Based Voucher recertification. Please sign and date electronic documents and, upload or send all income and other verification documents to your Housing Specialist.
ALL household members who are/will be 18 years or older MUST sign all paperwork and provide ALL income documentation.
*REQUIRED FOR NEW FAMILY MEMBERS OR ANYONE TURNING 18 YEARS OLD.

Government issued identification for each NEW adult member or anyone turning 18 years old.

Birth certificate for each NEW household member. (Must contain seal and registrar’s signature).

Social Security cards for each NEW household member.

Marriage certificate, separation papers or divorce decree. (If last name is different from birth certificate).
ALL documents, unless otherwise specified, MUST BE DATED WITHIN THE PAST 60 DAYS; screenshots, photos of documents, faxes, and emails are acceptable if it contains your name, is fully legible, is the complete document containing all pages.
Proof of Utilities - A copy of your most recent utility bill for BGE, water, oil, propane, etc.
Income

Most recent Social Security award letter. (Current year). Contact ssa.gov to get a copy.

Department of Social Services benefit statement for TCA, TDAP, TEHMA and Food Stamps (all pages).

Employment letter on company letterhead advising start date, wage and hours (for any new employment).

Two (2) most recent consecutive paystubs if currently employed.

Self-employment - Copy of last year’s 1040 tax form and Schedule C or, last year’s receipts or tax return AND notarized self-affidavit of declared income.

Most recent quarterly pension benefit letter with monthly payment and deduction amounts.

Unemployment benefit letter stating the weekly benefit amount (must include all pages).

Workman’s Compensation award letter stating the amount of monthly payments.

Child support print-out for the past 12 months from www.dhr.maryland.gov/childsupport-services and/or a contempt filing OR;
• Notarized statement of child support from absent parent (must include child’s name, payee’s name, address and phone number). -
Notarized statement from anyone providing support to meet your living expenses, must include:

The dollar value of support, how often support is given, reason for support, when support will end.

Name, address, phone number and email address of provider and; must be signed and dated.

Zero Income Statement if claiming no income for the household, followed by an in-person Zero Income Questionnaire interview with your Housing Specialist. (May be required on a quarterly basis).
Assets
-
Copy of two (2) most recent bank statements (not an account summary printout) for checking/debit accounts such as Direct Express, Cash App, PayPal, Zelle, Chime, Credit Karma or Venmo (all pages/page numbers must be visible).

Copy of most recent and savings statement. (all pages)

Copy of statements for money market or certificate of deposit (CD).

Copy of life insurance policy and verification of cash value.
-
Copy of deed or mortgage for any owned property or business.
Expenses
-
Verification of childcare expenses for children 12 years old or younger, if adult is working or attending school. (If receiving daycare vouchers, provide documentation of co-pay amount).

A notarized letter from Daycare provider with payment amount and child(ren)’s name(s).

A notarized letter from a licensed Daycare provider, on letterhead.

Medical expenses for families with a Head of Household who is 62 or older or disabled:

Documents for anticipated medical expenses for 12 months that will not be paid by an outside source or insurance.

Medical expenses approved by the IRS.

Statements or bills from medical professionals for unpaid balances or co-pays. (Large medical bills must show proof of making consistent payments or proof of payment agreement to pay the balance).

Medicare and other insurance statements and proof to support current payment.

Prescription receipts or pharmacy printout for the past twelve 12 showing the patient responsibility balance.

Student Status
-
Verification from the Admissions or Registrar’s Office of any adult household member, who will be18 years of age or older, who is claiming full-time or part-time student status. (School records, such as paid fee statements that show a sufficient number of credits to be considered a full-time student by the educational institution attended).

Proof of financial aid amount received and/or paid to an institution.
