Checklist for Housing Application

PLEASE BE SURE TO PROVIDE OUR AGENCY WITH ALL OF THE REQUESTED INFORMATION BELOW. PLEASE HAVE ALL HAND WRITTEN LETTERS NOTARIZED PRIOR TO SUBMITTING TO OUR AGENCY
 
  • Birth Certificates for all members. Birth Registrations are not acceptable

  • Married Couples (your marriage License)

    • Marriage License if your last name differs from that on your Birth Certificate

  • Social Security Cards for all members

  • Photo I.D. for every family member of the age of 18 years

  • Current Lease or Notarized letter from landlord/family member stating full address of unit, beginning of lease, or date of move-in, amount of rent plus utilities paid each month, and the living agreement between the two of you. PLEASE DO NOT BRING IN RENT RECEIPTS ONLY YOUR LEASE OR NOTARIZED LETTER FROM LANDLORD IS ACCEPTABLE! NO EXCEPTIONS! Utility Bill (Copy of gas/electric, and water bill if it is in your name)
     

ALL INCOME VERIFICATIONS MUST CURRENT AND DATED WITHIN 30 DAYS AND BE PROVIDED FOR EVERY HOUSEHOLD MEMBER 18 YEARS OR OLDER
 

Social Security or SSI award letter(s) (Current award letter within 30days of invite to request a new one go to ssa.gov or call 1-800-772-1213)

  1. Pension (Veterans, Retirement, Widows)
  2. Workmen’s Compensation Verification Letter
  3. Dept. of Social Services print out of assistance (TCA, FOOD STAMPS etc.)
  4. Unemployment print out
  5. Child Support verification from Domestic Relations even if you do not receive payments, or if support is paid directly to you a Notarized letter from Child Support Payee (stating monthly amount paid, and name(s) of the child(ren) in which the support is for.) You may obtain a printout summary online if your case is through MD Child Support Enforcement. Website address: www.dhr.maryland.gov/childsupport-services
  6. Notarized letter(s) from family members that contribute money to your household stating the monthly amount that is contributed.
  7. All other income received in the household regardless of age.
  8. Two (2) months of Bank Statements (All pages). This includes account from Chime, Direct Express, etc. Call the phone number on the back of your debit card to order if it is not a bank.
     

Letter from your Employer on their Letterhead or Official Company Stamp dated within (30) days signed by someone of authority. Letter needs to State the following:
 

  1. Start Date

  2. Hours worked per week (If your hours are flexible, please have your employer state an average number of hours.)

  3. Hourly Wage

  4. Pay period (weekly, bi-weekly, etc.)

  5. 2 of your most recent pay stubs
     

  • Asset verification – any accounts in addition to your checking and savings

  • Homeless Letter from State Agency, or Shelter, stating when you became homeless, your living arrangements, and why you are homeless.

  • School Verification for Self and/or all Minor Children (On school letterhead and/or Report cards for current year)

  • Childcare/Daycare Verification: (Notarized letter, or letter on company letterhead, stating name/s of children being cared for, and the monthly amount charged for the care.)

Please note that you will have to upload ALL of these items to your portal at myaccount.hcaac.com
